Franklin House Dental PlanYour choice for dental care with usAs individuals we have to make more and more provision for our own health needs. Franklin House Dental Practice believes that the best way of making treatment affordable without compromising our high quality of care is to offer membership of the Franklin House Dental Practice Care Plan/Child Care Plan, which provides the following benefits: • A convenient payment scheme for routine treatment Your child's dental careChildren are born free of dental disease, yet by adulthood 95% of the population has active gum disease and most people have experienced some tooth decay and even tooth loss. It is our belief that disease can be prevented. Our aim is to provide your child with the best dental care available to secure their dental health. We ask parents to register their children from birth so that we can see them twice a year to help them grow up free from dental disease. However, if they do need treatment, we can offer them alternatives not generally available on the NHS, such as mercury-free white fillings. To the children of members of our Franklin House Dental Practice Care Plan/Child’s Plan we can offer these alternatives on the same terms as their parents. How do I register?Please call in at the practice where an agreement will be completed. This will require your signature and bank account details for the Direct Debit Mandate. Direct debits are collected form your account on the 8th of every month. Any patient enrolling before the 27th of each month will benefit from cover from the 8th of the following month. If you have any queries, please ask any member of staff. The practice care plan - £14.75 per monthRegular dental appointments (two per year) to provide: Regular hygiene appointments (two per year) to provide: Assessment of emergencies and dental pain carried out at the practice during normal surgery hours. All treatment planning for your future dental needs 20% reduction off any fillings 20% reduction off any laboratory-based treatments required, i.e. crowns, bridges and dentures. 20% reduction off any additional hygiene work Extended Payment Scheme available for treatments over £500 Worldwide Trauma insurance to protect against the cost of large unforeseen accidental damage Emergency Call-Out insurance, should you need a dentist in an emergency, anywhere in the world Redundancy Protection for up to 12 months. The child care plan- £7.50 per monthOur Child Care Plan has been developed to ensure that your child receives all the preventative techniques currently available. Children on the plan receive regular oral education with two dental examinations, one oral hygiene instruction, one hygienist appointment per year and fissure seals where necessary. All the benefits of the Practice Care Plan are also available. Extended payment planFor any plan members who require additional treatment costing more than £500, we offer the convenience of being able to spread the payment over 10 months, by simply adding the fee to the existing direct debit, Please note our normal discount rate is reduced from 20% to !0% when the Extended Payment Scheme is used. A deposit may be required. Use of the Extended Payment Scheme is entirely at the discretion of the dentist. Our staff will be pleased to discuss the details with you. Worldwide trauma & emergency call-out insuranceEmergency Call-Out cover can be provided by any dentist worldwide who agrees to treat you. The cover is for a maximum of £120 per claim (outside the UK £320) for pain relief with and excess payable of £25. The total amount reclaimable for call-outs in the UK is £95 and £295 outside the UK. You simply pay the emergency call-out charge to the dentist concerned and collect a receipted invoice. This will be processed through the practice for re-imbursement from the insurers. This is to provide emergency treatment only. You should return to the practice in normal hours for any further treatment required, provision of which may be already included in the Plan or may qualify for a discount. Dental Trauma – if you are unfortunate enough to suffer a dental trauma, for example, as a result of a road traffic accident or an accident at home or work, the insurance cover is in place for a maximum of £5,000 with a £125 excess applicable for internal trauma and a £25 excess for external trauma. In addition, should you have to stay in hospital as a result of a dental trauma, you will be entitled to claim £50 per day, after the first day. If you are scarred on the face or neck as a result of the accident, you will be entitled to compensation from £50 to £500. Oral Cancer – should oral cancer be diagnosed, you will receive a payment of £2,500. Redundancy Protection (not included in the Registration and Insurance Scheme) – plan premiums paid for up to one year, with a waiting period of 30 days. (Up to a maximum of £12.50 per month). Details of the insurance cover and exclusions are available at the surgery. Registration & insurance schemeIf you would prefer to pay on a fee-per-item basis, we strongly recommend
that you use our Registration and Insurance Scheme. Registration with
the practice costs £25 per year and will include the insurance cover
described above (except Redundancy Protection). It is, however, conditional
upon you attending the practice at least once a year for a check-up and
having any remedial work undertaken as soon as possible, payment for which
is not covered. |
